A good example of a letter of good health

Doctors are the ones that write letters of good health at the request of patients. Patients seek for such letters for various reasons.

Among the main reasons why patients request such letters are for example paperwork to obtain a driver’s license, for reasons of travel, this could also be a prerequisite to entering a new job, requirements for their wedding, custody of minors, among many others.

So the process is simple. First the designated doctor will conduct a general health check of the patient. This is the same doctor who would write the corresponding letter, which must have a visa form the health center in which the health check was done.

Example No. 1 of a letter of good health:

This certificate is issued and stamped with my signature approved by this health center. Mr. Claudio Garcia Leiva can use this letter for the purpose he would like to.

Through this paper we wish to certify that Ms. Luisa Vaca Quiñones, age 37, residing at Avenida Las Delicias 988, Buenos Aires and with ID No. 12459100 has been subjected to a general medical examination having obtained a very satisfactory result.

Ms. Luisa Vaca Quiñones underwent several medical tests in this hospital, SAN LEONARDO HOSPITAL, on 14 September 2011. The results of these tests allow us to assert full good health. This letter can be use for what Ms. Luisa Vaca Quiñones considers appropriate.

Through this letter I state that Mr. Miguel Andres Ruiz De Las Casas, age 28, with identity card No. 10247810 and Venezuelan citizen, passed all the medical tests he conducted at San Geronimo Hospital.

To obtain the driver’s license of private cars is by law required the approval of the tests A, B and C determined by the Ministry of Health, in this case, Mr. Miguel Andres Ruiz De Las Casas passed all three.

Therefore, it is stated that Mr. Miguel Andres Ruiz De Las Casas passed the medical examination necessary for obtaining his driver’s license and it is valid for 6 months beginning on March 28, 2011.
